  2. Attestation form to be signed by a Level 14 Officer in the State Civil Service or Honourable Member of the Oyo State House of Assembly or Local Government Chairman of the Applicant
  3. Parent/Guardian Consent Form
• Applicants must be Nigeria citizens by Birth
• Applicants must not be less than 1.66cm tall for male and 1.63cm for female
• Female Applicants must not be pregnant as at the time of recruitment.
• Applicants must be medically and physically fit.
• Applicants must be free of any conviction(s) on criminal ground, by a court of law.

The Applicants that meet the requirements from the online registration would be invited for physical screening at the Authority's operational Headquarters, Sanyo, Ibadan at a date to be communicated.

A candidates who is suffering from any of the under listed abnormalities or deformities shall not be accepted for enlistment into the Authority.

Applicants applying as sportsmen/women are to present evidence of their professional experience with certificate and medals of attendance or participation.

• Impediment in speech.
• Gross malformation of teeth or jaw preventing proper mastication of food.
• Knock knees, Bow legs, Amputation of any part of the body.
• Bent knees that cannot be straightened when standing at attention
• Deformed hands which cannot perform the full functions of the hand.
• Defective eyesight or squints eyes.
